HEART = Observations
An unsafe observation is a circumstance in which there is a behaviour or condition that could potentially result in an adverse event unless action is taken to prevent it. Observations and the action taken to prevent an adverse event should be recorded in HEART. You can also submit safe observations and the reinforcement action taken to praise or encourage continuation of good practice.
CAIRS = Incidents/Near Miss
An Incident or Near Miss is a circumstance where an event takes place and does, or almost (near miss), results in an adverse event. These must be reported direct to your line manager & local HSSE contact for inclusion in CAIRS.
